are you working locally or on the cloud instance? It looks to me as if the ports are not yet forwarded:
kubectl -n cass-operator port-forward --address 0.0.0.0 service/grafana-service 3000:3000& kubectl -n cass-operator port-forward --address 0.0.0.0 service/prometheus-operated 9090:9090&
